To complete a referral, you will need the carer’s:
- Name
- Address
- Contact details (phone/email)
- Brief information about their caring situation
If you are referring on behalf of a carer, please ensure the carer is fully informed about the referral and aware that someone from VOCAL will contact them.
Once a referral or self-referral is received, carers will receive a comprehensive Welcome Booklet which includes information about caring, accessing support and VOCAL’s services. We will also speak with the carer to discuss their caring situation. The carer will then be matched with a dedicated carer support practitioner who can provide one-to-one support by phone, at home, or in the community, based on the carer’s preferences and needs.
